Supporting a Patient Centered Medical Home for youth experiencing chronic diseases

The Children's Hospital of Los Angeles (CHLA) had begun adopting a Patient Centered Medical Home (PCMH) model for several hundred youth who experience two or more chronic diseases. Their PCMH model focuses on coordinated care for these individuals who too often end up hospital emergency departments (ED) instead of receiving preventative speciality care.
Within CHLA are two divisions that provide services to an individual - inpatient care (managed by CHLA) and outpatient care (managed by Altamed). A youth experiencing multiple chronic diseases oftentimes is admitted to both inpatient and outpatient services for the same illness, but the patient data is kept in silos within each organization. To add to the complexity, each data silo is managed by two different electronic medical record (EMR) systems.
The problem with these data silos is that it prevents a caregiver from either CHLA or Altamed to fully understand the patient's medical history and provide whole care for the individual under the PCMH model.
The hypothesis was that if the data from the two EMR's were made interoperable on the backend and visualized on a powerful dashboard, then it would allow for caregivers to provide more a more efficient coordinated care plan, with the ultimate goal of decreasing the number of emergency department (ED) visits and increasing the number of PCMH visits.
